The City has partnered with PayForm.US to offer convenient online document completion and electronic payment processing.
To purchase a panel online, click the link below. Once the form has been completed, you will be prompted for payment. Completed forms will be sent to you and City representative for processing.
Payments processed electronically will be assessed a 3% Convenience Fee. There is a minimum Convenience Fee of $1.00 for all transactions under $33.00.
